Attention Parents:

              OMS Clinic will have a medication drop off for all students returning face to face. The drop off will be on Wednesday, August 26th from 5pm-6pm. If you are not available during this time, we will also be accepting medications during clinic office hours on Wednesday, August 26th, Thursday, August 27th, and Friday, August 28th by appointment only. Please contact the RN directly at [email protected] to make an appointment.

              Medication drop off instructions are as follows:

    All prescription medication must be in the original container, labeled, with the student's information. All over the counter medication must be in the original, unopened container. All medication must be presented with the medication administration form filled out by the physician and parent as well as the medication check in form filled out by the parent. We will not take any medication without these forms. Both of the forms below need to be printed and completed.  Please place all medications with forms inside a clear, ziploc bag with the student's name and grade labeled on the outside. 

Upon your arrival, please park, wear a mask, and come to the office door for drop off at your scheduled time.
